THE CHALLENGE

A Maryland-based Medicaid managed care organization (MCO) was on a path to provide their members with an improved patient experience and better outcomes - all at no cost to the member. Taking into consideration the chronic and complex conditions, financial fluidity and other characteristics inherent in their member population, the MCO sought a solution that would make it easier for members to access care outside of the Emergency Room (ER) or urgent care, close gaps specific to Healthcare Effectiveness Data and Information Set (HEDIS) measures, and differentiate themselves in the regional market. The MCO needed a new mode of care delivery that would allow their members to feel valued and empowered to take control of their care - all while reducing risk and cost.
